AB1303 AVASCULAR NECROSIS OF THE FEMORAL HEAD – NOT TO BE OVERLOOKED SEQUELA AFTER COVID 19 INFECTION

چکیده
Background COVID 19 infection could lead to different sequelae in survivors, known as post-COVID or long syndromes. Some of them are thought be due the thrombophylic changes observed infection, but some caused by administrated (especially high dose) corticosteroid treatment. Avascular necrosis femoral head (AVNFH) is a multifactorial disease which leads compromised vascular supply, ischemia and finally head. As corticosteroids usage states among main risk factors for development AVNFH [1], it presumed that frequency this will increase with pandemic. The exact dose needed not clear, has been stated higher daily larger total cumulative substantially osteonecrosis [2]. Objectives To describe detail characteristics diagnosed patients after infection. Methods study was done tertiary university rheumatological clinic. Data extracted from records who have referred clinic because hip pain between June December 2022. Inclusion criteria were: - new onset uni-or bilateral started documented infection; an MRI scan joints showing one both heads. Exclusion were presence prior anamnesis traumatic injuries hips pelvis, personal history hypercoagulable states. Results Nine (4 women 5 men) average age 59.1 years (range 38-72) included study. Four had five – unilateral AVNFH, thus 13 analysed (8 left right sided). mean time lap start 26.2 weeks 10-48 weeks). All limited painful movement their symptomatic hip(s), especially internal rotation four also elevated CRP levels (mean 11.7 mg/L). stage evaluated according Ficat-Arlet classification (0-IV stage). In I, classified II remaining III. exhibited effusion/synovitis on ultrasound examination corresponding scan. It should noted effusion found related worse prognosis [1]. three amount required arthrocentesis fluid aspiration. analysis joint consistent degenerative (i.e., low WBC count predominant lymphocytes no crystals). our received during COVID19 while 6 hospitalized more severe disease. According patients’ documentation, 936.2 mg prednisolone equivalent per patient 187-2272 mg). Conclusion must overlooked Our results show suggestive osteonecrosis, they registered all patients. these two necessitates referral hips, order detected timely. Avascular Necrosis of the Femoral Head: Are Any Genes Involved?
Avascular necrosis of the femoral head (ANFH) is a pathologic process that results from interruption of blood supply to the femur bone resulting in the death of bone cells and collapse of the femoral head. Nontraumatic ANFH continuesto be a significant challenge to orthopedic surgeons. While the exact mechanisms remain elusive, many new insights have emerged from research in the last decade tha...
متن کاملAvascular necrosis of the femoral head.
The patient was a 51-year-old man who was evaluated by a physical therapist for a chief complaint of right hip pain. The patient was treated with a single bout of nonthrust hip joint mobilizations, but due to the patient's atypical response and long-term use of oral corticosteroid medications, he was referred to his primary care physician. Radiographs and subsequent magnetic resonance imaging r...
متن کامل[Avascular necrosis of the femoral head after transcervical fracture].
The association of transcervical fractures of the femur with avascular necrosis of the capital fragment was recognised many years ago. Nevertheless several basic questions, such as the incidence ofavascular necrosis, the extent and pattern ofbone death and revascularisation and also the role played by avascular necrosis in non-union of these fractures, remain unanswered. In the hope that a hist...

Since the recent publication of studies of the vascular pattern of the proximal femoral epiphysis in man by Tucker (1949), Trueta and Harrison (1953), Judet, Judet, Lagrange and Dunoyer (1955), and Trueta (1957) there has been a renewed interest in the mechanism of capital necrosis after fractures of the femoral neck. The literature on the subject is so extensive that no effort will be made her...
متن کاملComparison of Total Hip Arthroplasty and Core Decompression in Avascular Necrosis of the Femoral Head after Taking Temgesic and Norgesic
Background: Osteonecrosis is common after taking Temgesic and Norgesic. This study intended to suggest the preferable surgical treatment in osteonecrosis patients who use these drugs. Methods: Twenty tree patients with hip osteonecrosis who were hospitalized in the Kerman hospitals after using Temgesic and Norgesic were followed for about 14 months from 2008 to 2010 (from a minimum of 2 months ...
